I never take Ambien anymore (ambien+whiskey caused me to sleep walk and stop at 3 food trucks around 2 AM one night in Portland, didn’t remember it at all) but I woke up wide awake at 3 AM last night, and at 4 decided to pop one to get some ZZZs. Luckily nothing but a dozen check ups this morning because it’s still in my system and I can’t keep my eyes open.
I saw a 350-pound man take an Ambien while drunk and about 20 minutes later pass out standing up onto a table. Said table groaned agonizingly as it took on his weight, rallied, and then sprung back, which created catapult-like propulsion of his massive frame that resulted in him being flung onto the floor, at which point he didn't awake at all. The utter determination of this table not to succumb to his mollyhog mass and maintain its integrity still inspires me in these difficult times.

Don't see covid going anywhere between now and next spring so if you don't play now, schools better be prepared to not play at all. Not a financial hit that most ADs are going to be super eager to take, so I'd imagine the P5 schools will move hell and high water to get as many games in as they can.

Acetaminophen. It's all a kid needs for even the most intense post-op pain control. And evertim you got a real old-timmer with terrible pain give em one regular strength 'lenol and they're all "wow, doc, what was that stuff?" 3. Triamcinolone 0.1% cream. Two kinds of rashes in the world: "try this cream" and "go see a dermatologist and they'll prolly still tell you to try this cream." T4. Combined oral contraceptives and Plan B. Does not need to be explained and has gotten ol pnkskrt out of a good many tight spots 5. Ozempic. Had to make room for a relative newcomer. Honorable mention: metformin, varenicline, prozac, miralax, torsemide, buprenorphine ( probably the Bama that is too low)

Glipizide. I spend half of my days taking patients off the glipizide my older colleagues put everyone on. 5. Amoxicillin. Not a fair pick, cuz it's on the list because of its overuse and the aneurysm in my brain that swells evertim DirtBall tells his toilet customers they need it before their mouth toilet cleaning because they had a knee replacement 5 years ago. Dishonorable mention: tramadol, aricept, atenalol, testosterone (for "low energy" not trans stuff)
Don't even get me going on this topic. Most all the sleep aids are garbage. Fuck the old timer docs putting their 87 year olds on 2 mg of Klonipin for sleep. Not big on muscle relaxants either. Glipizide is at least better than glyburide. Throw Actos into the mix. I've yet to hear of a single redeeming quality of atenolol. Others that suck: 1. Captopril 2. Demerol 3. Warfarin 4. Depo Provera 5. Amiodarone (effective but just a train wreck of a drug) 6. Tamiflu Best drugs on the market today not already listed 1. Rocephin. Has anyone tried sprinkling some 'cephin on the 'rona patients? 2. Metformin 3. Lipitor 4. Ketamine. Very underrated drug. Farxiga and eliquis will make the list once they go generic.

I would put azithromycin over Amox for similar reasons (bUt mY oThEr DoC gIvEs iT fOr mY CoLdS!?!?) And warfarin too valuable to be on this list. Yeah yeah yeah AKI and gastritis but the shit works like tits. Sure it's got high overuse potential, but it's overused cuz it works. And think about the range of indications for NSAIDs. It's crazy. Yep, if I could only ever use 5 meds in practice, ibuprofen is absolutely one of em. GLP-1>SGLT2 especially ozempic/trulicity cuz it's been a life saver for my poorly controlled diabetics who can't manage meds for shit. Weekly injection is the GOAT. Also, I've seen that Fournier's happen to someone that was on an sglt2, not sure about causation, but I'll never not see that when trying to fall asleep at night again. That's on me though Prozac slightly cheaper than sertraline, better studied in adolescents and that self-taper is nice. But I agree and I prescribe zoloft more. Metformin dangles off the list by a thread just because I've got too many patients that bitch about it and can't manage to get or stay on it. Kinda similar case with atorvastatin, and that primary prevention NNT is unsexy enough to keep it out of the 5. Zetia yeah sorry ACC but I'm never putting someone on zetia.
