Ive seen both, but mine is handwritten. When I went for the first dose the person didn’t even fill my DOB out lol so I had the lady fill it in when I went the 2nd time.
If you have a comprehensive list of what these are then please post them, I can only find tidbits from articles about what the FDA said about "high risk" but nothing concrete from the CDC.
