Agree with this - but being realistic I'm assuming there was negligence at some level involved here. This just doesn't happen - and there are a shitload of protections to make sure it doesn't happen.

I feel so awful for the families. You would never expect something to happen like that on a set. Just a nightmare.

There are so many checks and balances on sets to prevent this very thing. I handle guns on sets often, and have never encountered anything remotely close to a prop gun that would be capable of killing someone. Blanks, half or quarter loads for noise, and shell casings are basically the extent of it. I can’t wrap my head around this.
With all the terrible movies that have come out over the years where you question if there was anyone with a brain on set it seems like it'd happen more often.
I watched a 30 minute horror movie on Youtube that had a $23k budget and they could afford a firearms trainer/handler/however you want to describe it.
Much more likely that a misfire left fragments that were projected out by the next shot. This was likely a close up shot, where they used a half load or potentially full load (just referring to the amount of powder), to make it look big in front of the camera. High levels of powder obviously would push any potential projectiles out faster. The prop master likely fucked up, and didn’t check the barrel between takes. Even with the prop master’s fuck up, the line of sight from the gun is always cleared of people. You also never aim directly at people, regardless of the shot type. You can aim slightly off of camera and it still reads as if you are aiming directly at it.
