    I highly doubt the virus is actually any worse. Infections among particularly young people are way up. Virtually certain this is why there are more severe cases.

    Young people were always ending up in the ICU.

    Gotcha...a couple doctors on tv today were saying the cases they are seeing are more severe than say 8 weeks ago, but one did mention that younger people are more likely to wait to go to the hospital. That certainly has to play a role, but I wonder if there are other factors at play like incresed viral load from gatherings in enclosed places (bars/restaurants) with recirculating air.

    Yes. We are testing more “low risk“ people. These are absolutely cases that were happening in March and April but we had shitty testing capacity, and so march/April was probably much, much, much worse than our numbers looked, and that’s why deaths (and CFR) were higher.

    yes, young people die much less often and almost entirely have mild symptoms (except when they don’t).

    this surge isn’t a big deal as deaths and capacity is still available (for now) ASSSSSSUUUUUUUMMMMING these young people don’t spread to old people.

    if they don’t, the rise of cases won’t amount to much, but it’s way too early to spike the football yet.


    Also this douchebag is speaking secondhand hearsay about hospitals he’s not affiliated with. I’m involved in the administration of a chain of hospitals and have friends associated with hospital, ER, and ICU care and NOOOO ONE is “inventing Covid” cases to boost reimbursement. That’s fucking fraud, and obviously this will all be audited heavily. He pretends like testing For Covid in inpatients in the hospital for other conditions Is a bad thing, trying to cook the books.

    the hospitals are protecting their nurses and staff from incidental Covid positive cases. Jesus this guy sounds like a moron. Oh wait, hospital administrator, guess it fits.

    No good data to support the viral load correlating to illness severity phenomenon yet. Hasn't stopped p much all of my colleagues repeating that unsupported theory to date, though.

    Delaying care absolutely has implications for outcome though.

    I don't know how cases could be more severe. We already saw people in their 20s and 30s with ARDS. With stroke. With sudden death from saddle PE. With massive, and I mean massive, coronary thrombosis. I'd expect there to be more of those sorts of cases among young people as the denominator increases, but it's not like the severe cases are somehow getting more severe. Not sure how they could. They get in a car wreck on the way to the hospital?

    In MN, new cases among 20-29 year olds is about quadruple what it was a month ago. And overall # of cases continue to increase modestly, yet hospitalizations keep going down.

    I think ED and ICU docs see ED and ICU patients. They might "see more and more severe cases among young people" but it's probably because they see only the severe cases among young people. Without seeing good data, I'm disinclined to think young people are, by percentage, getting more sick.

    Thanks for your detailed response. My uneducated guess after reading that is younger people are probably waiting too late to seek treatment and therefore allowing the virus to do more damage in the meantime. Older patients would seem more likely to immediately seek help knowing how much age plays a factor in mortality rate.

    Maybe. There are tons of older pts delaying care. I wouldn't bet on there being much of an age related phenomenon there

    There's selection bias in ICU #s, too. Lots of older folks declining ICU cares, opting for comfort. Younger patients more likely to cautiously get sent to ICU? Many younger pts in the ICU not on ventilators, may speak to that.

    So, I told my wife in February that by the time this is over we will know someone who died of this. Yesterday she tells me that I probably over reacted when I said that. Then I said, one of your best friends grandfathers died... she said, well I never met him. Me: facepalm. Then I had to explain how this is far from over. We are just starting. I think she understood but even she is getting the “I just want this to end” feelings.

    I've lost most of my faith in urgent cares. I was having a text book (not that I read the book, but a doctor should have) migraine with aura, went to urgent care, and was diagnosed with dehydration and low blood sugar (despite finding my blood sugar levels were normal)
    My wife went to urgent care a few years back after smashing her toe. The took x-rays and said it was a bad sprain. After a week of hobbling around she went to her doc, where they diagnosed a broken toe, using the same x-rays urgent care took.
    There was another one where my daughter was misdiagnosed a well, but I forget what it was. Where do they get these urgent care docs??

    one of the big reasons I love tmb is that, on average, it's a pretty bright, hilarious and highly functioning online community. So we can afford to cut a level beyond broad public health messaging and trust that it will be responsibly received.

    Feel like I gotta say a thing about UCs even though yes they're guilty of a lot of shit medicine, but I got a couple buddies who work at them sometimes, and you gotta realize that they get absolutely cannibalized the few occasions they make a "less is more" call and end up wrong. Had a buddy get absolutely eviscerated by our primary care group because he deferred a chest CT on someone with almost no risk factors for PE, one mild symptom, and a completely normal set of vitals and exam, who later ended up hospitalized.

    And a lot of people get a lot of poorly evidenced shit given to them cuz this is America and everyone expects a cure in hand today and will not accept "don't think there's anything to worry about here, go home and eat soup and call your primary if you get worse"

    Nobody likes a Monday morning MD.

    those aren’t false from his POV, but are in general completely false statements.

    when you are a specialist you tend to forget what the average patient looks like and what is running around out in the real world.

    The virus is neither stronger nor weaker. In Utah over 30% of covid ICU patients are in their 30s and 40s. The UDOH says that’s because Utah is demographically the youngest State and the 30-40 groups are the ones currently contracting the disease. That’s probably correct. However it flies in the face of the conventional wisdom from a month ago. Luckily almost all of them will survive.

    Viral load causality with the severity of symptoms is documented with other viruses. Just because it hasn’t yet been yet proven with C19 doesn’t mean it isn’t a factor.

    Yeah if I hadn't seen the second Dr say he was seeing increased severity (can't find the video, he was on with Tapper and out of Miami) I wouldn't have asked, but two Docs saying it made me raise my eyebrows a little. Hence why I came to my trusted source. :respek:

    People won't even wear a freaking mask out of respect for others, let alone their own protection. I'm afraid the nuckledraggers will decide they don't need the flu shot either, especially the more they're told they need it by the media and experts...hope I'm wrong. I know you're not specifically speaking of the shot, but that's just one variable that comes to mind.

    sure. But the whole thought arose with this virus when people supposed it must surely be what leads to a disproportionate mortality and severity rate among young HCWs. Until we found out there's no such thing as disproportionate mortality or severity among young HCWs.

    If you get tested at a hospital do you have to be admitted? My boss is claiming his friend is a respiratory therapist and they’re testing people just to admit them to get money.
    What’s best strategy for calling him out?

    That’s not a thing. A hospital admission is thousands and thousands of dollars and admitting someone with no symptoms just for a test is fraud. They’d get all charges rejected from insurances instantly.

    for an admission you have to have full admission orders and a doctor has to do an admission history and physical. No one has the resources to do that for every single test.

    that may be the single dumbest conspiracy theory yet
