Baylor is the fucking worst. They introduced private insurance to the United States at a time when public health options were in vogue throughout Europe. Germany had just spent the previous twenty years installing a universal healthcare system on the heels of von Bismarck's "healthcare is a human right" campaign. Baylor University Hospital offered select teacher's unions health insurance to fill the beds in their empty facilities. The teachers were chosen as a result of an in house study on professions least likely to seek medical attention if it were readily available. Baylor made sure to stay away from careers involving manual labor and increased risks of trauma/bodily injury. Health insurance was a fairly new concept at the time so Baylor used annual expenditures on cosmetics and nylons to sell coverage. "You wouldn't buy all of your makeup in a day, so why wait to pay for your hospital visit"?
Baylor isn't the only agent involved in birthing our abomination of a healthcare system, but they sure as shit got the ball rolling.

Doctors and the AMA pushed for the growth of hospitals and standardization of care and equipment. The costs associated with moving medicine out of the home (or small clinic) into large modern facilities were passed to the patients. Most hospitals remained empty and were actively pursuing alternative revenue streams. The onset of The Great Depression accelerated the issue.
AMA's involvement in lobbying against government-funded health options. This included free health clinics and a lot of nefarious shit that would make the Freedom Caucus uneasy.
Salary caps enforced during World War 2 made benefits packages one of the few ways companies could actively attract labor.
Legislation and court rulings allowing corporate deductions on employee healthcare costs.
Labor relations and the outlawing of "wildcat" strikes. If you were a union worker, you could not strike without prior approval from union leadership IF you had signed a collective bargaining agreement. Creative legislation categorized employer based healthcare packages as valid CBAs.
