I mean, most states have databases. It takes ~5 seconds to pull mine up. I’d especially expect a state like Hawaii to be extra cautious.

We just got back from Hawaii (used our real vaccination cards), and they are diligent. Prior to arriving, you have to create an account on their official travel site, where you either upload your vaccination card or a negative test taken no more than 72 hours prior to the last leg of your flight there. They review that and send a QR code. Then, either before you board your last flight (depending on airline) or upon arrival in Hawaii, they check your QR code and the physical copy of your vaccination card. I would assume she was pretty easily caught.
There’s nothing wrong with it being five weeks apart. It’s not a requirement they be exactly four weeks apart for Moderna. We’ve had some people at my clinic go months between shots. And both shots being the same lot # is also not impossible. It doesn’t happen often, but it’s happened at my clinic. These vials last months if they stay unpunctured. When vaccines at my clinic slowed down, we did Moderna first doses at like say July 1 with lot abc123. Didn’t do anymore until July 28 for their second doses and we still had lot abc123 in stock. Each vial doesn’t have its own lot number.

I was laying in bed last night dumbfounded at the absurdity of these anti-vaxxers. The same people who've shouted "Covid shouldn't dominate your life! Don't make such a big deal out of it, it's just the flu!" would rather eat horse goo and risk a prolonged hospital stay and death rather than take 30 minutes to get a vaccine that will let them move on with their lives as if Covid really is the flu. The more you deny Covid's potency, the greater risk you run of allowing it to dominate your life.
