DirtBall you're one of the dentists here right? I got a regular checkup today and need a new mouthguard and this is the second time I've heard about really seeing if getting some ortho/invisalign kind of thing would be better long term to possibly help with sleep apnea (ortho today said my long uvula could be causing some sleep issues). It's a cheap cost to wear some special ring at night that measures my O2 level and I guess if it's fine then they'll just do a night guard. Anyways have you had to educate yourself a lot on this topic now? Seems like even three years ago I didn't hear anything about sleep/tongue/uvula problems from my dentist and now it's like all of them got some sort of official marching orders from the ADA.
Sleep medicine and especially dental sleep medicine has come a long ways over the past 5-10 years. It can and does make a huge difference in people’s lives. And clearly it’s much easier to sleep with a specialty mouthpiece than a CPAP. There’s still some murkiness on if a sleep study (and which type) is required, some insurances require one, some state boards require it, some don’t. My board requires an overnight study for an “apnea device” but not for a “snore guard”. They both open the airway but if I make a snore guard for someone with apnea or an apnea device for someone without and don’t have a sleep study there’s liability. I did an 8 hour CE with a lady from Denver and they can do the at home test (I think that’s what your doc is talking about) and that counts. Here even with the at home O2 test I’d still need an overnight clinical study so I only make those devices for friends/family. Both my parents have devices and it’s made a huge difference for both of them. Sit on the couch and hold your breath for 20 seconds every couple minutes and see how you’re feeling after 10 or so minutes, that’s what’s happening all night long. So if you have apnea and can treat it with an oral appliance, 100% do it. (Also an FYI, an apnea device is much more expensive than a plain night guard) They ortho/invisalign to help with apnea….ehh I’m not as thrilled with that. Getting your teeth into proper occlusion makes a huge difference for the teeth, gums, and surrounding bone. It’s all about evening out the forces applied to each tooth. For people with a deep bite it will open the airway some. For people that clench/grind bad it can relieve those symptoms and that definitely would help with apnea/snoring. But I never promise that to a patient because I’ve had tons get aligned but they still have stressful lives and clinch every night. So yes that’s very beneficial but it may not make a huge difference for the apnea. One final PSA, for anyone that wakes up with a tight jaw, if it clicks/pops, if you have chipped teeth, gum recession, or see little divots in your molars, go to your dentist and get a professional guard made. I’ve gotten to the point where I only charge $250 for them (used to charge 500, but all it takes to make one is a 2 minute scan done by an assistant and a local lab makes them for me for $40). Again, it equilibrates the forces in your mouth and it keeps space between your teeth which puts your TMJ into proper position. In your teeth touch all night the bone in the TMJ sit on each other and it will cause the squishy protective disc to displace itself. Once it’s gone it’s gone and you’re stuck with jaw issues. For me it protects the crowns and fillings I do so patients don’t come back bitching in a year or two because my “work was crappy”, no the work was fine but the reason that tooth needed a crown was because you ground it down and broke it, you didn’t want to protect that $1000 crown with a $250 guard so now you need a new one.
This makes sense. I don't foresee myself not clinching/grinding at night, if that's become my habit, just because my teeth are more aligned. Like you mentioned, the idea is to get the airway more open and maybe aligning my teeth will help, but it's not like it'll change the physiology of the other parts of my mouth and throat. I'd like to get a mouth guard after this regardless. I don't feel like I sleep that poorly, it's more of making sure I try to get at least 8 hours a night and i do a very poor job at that.
I’m always floored by just how different nfl games look on tv vs college. I just mean how it actually appears, not the difference in skill, size etc. The colors and image quality is just something else when the nfl comes on

My kid went to a basketball camp this summer with a bunch of the Husker basketball players and he got a mini basketball autographed by all of them. I was shocked at how few of them could sign their name in cursive, but then I thought about it and they don't teach it in school so it makes sense. Like 2 out of 10 guys could put together a decent looking signature, the rest just wrote their names.
I switched elementary schools between 2nd and 3rd grade. Old school taught it in 3rd grade and my new one taught it in 2nd so I just missed out and never really properly learned other than being able to rough out a signature.

it's mostly internet people so who knows how serious the pushback is, but it's a very vocal group and they kind of have a point. it's an insult to the legacy edit: apparently she also supported Danny Masterson during his rape trial? The Mars Volta's lead singer (whose wife was a victim) said this:
I had no idea who she is, but the Internet was pretty quick to find out she's a Scientologist and was at Masterson's rape trial to support him. I have to think they vetted her and don't give a shit.
Generally my take on this is that it’s a case where if the band asked her to be there and it makes them whole and they can enjoy playing music again, then who am I to say otherwise. This isn’t like they recruited a convicted criminal/abuser/metoo dipshit. honestly Scientology isn’t probably not even on the shortlist for most people or know why it’s a problem. Most probably know it as that weird Tom cruise thing. I think it passes pretty quickly.
